Location: Carmichael, CA

Start Date: 07-07-2026

End Date: 10-06-2026

Contract Type: Contract

Shift Type: 8-hour day shifts, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, Monday-Friday with rotating weekends

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Develop and manage daily patient lists, reviewing charts and assessing patient needs
Open goals of care discussions and educate patients and families on disease process
Facilitate advance directives and POLSTs, and hold family meetings as needed
Provide end-of-life discussions and support, attending daily rounds
Collaborate with a multidisciplinary team to coordinate palliative care across units

Qualifications

BLS certification required
2-5 years of recent experience in palliative and/or hospice setting
ICU or oncology experience is a plus
Strong acute care, clinical thinking, and communication skills
Flexibility and strong organizational skills are a must
Desire to serve patients, families, and team members

Knowledge & Skills

Palliative Care and End of Life Care principles
Acute Care assessment and management
Cerner EMR proficiency
ELNEC certification preferred
Goals of care discussions and advance care planning
