Contract Type: Contract

Shift Type: Weekday shifts, 8:00 AM - 5:00 PM; every other weekend required

Hours per Week: 40

Candidate Pay Rate: $82 per hour

Roles & Responsibilities

Conduct initial assessments and ongoing care coordination for a caseload of 15-20 patients daily
Perform utilization review using Interqual criteria to ensure appropriate level of care
Develop and implement discharge plans in collaboration with the healthcare team
Communicate with payers, uninsured patients, and homeless populations to navigate care
Document all activities in Cerner EHR accurately and timely
Adapt to changing priorities and reprioritize tasks throughout the day

Qualifications

Active California RN license in good standing
Prior recent experience at a Dignity Health or CommonSpirit facility required
Basic Life Support (BLS) certification from the American Heart Association
Minimum 2 supervisory references (charge nurse accepted) – one within 12 months and one within 3 years

Knowledge & Skills

Expertise in case management, utilization review, and discharge planning
Proficient with Interqual criteria and Cerner EHR
Strong collaboration, teamwork, and effective communication skills
Ability to work with diverse patient populations including uninsured and homeless
Excellent organizational and prioritization abilities
