Roles & Responsibilities

• Provide thorough outpatient physical therapy evaluations and assessments to develop individualized treatment plans
• Manage your caseload proactively with screening and case management to prioritize patient outcomes
• Document clinically sound progress notes and DME authorization/justification using NextGen EHR
• Conduct fall risk assessments and implement prevention strategies to keep patients safe
• Treat a wide range of orthopedic and neurologic conditions, from post-operative joint replacements to vestibular disorders
• Meet productivity expectations while preserving high-quality, patient-centered care
• Collaborate with multidisciplinary teams and communicate effectively with referring providers and support staff

Qualifications

• Licensed or eligible for licensure as a Physical Therapist in Indiana
• Previous outpatient physical therapy experience preferred
• Comfortable with productivity expectations (visits per day approximately 11)
• Experience or willingness to use NextGen EHR
• Knowledge of DME authorization and documentation of medical necessity
• Strong clinical skills in orthopedic and neurologic rehabilitation
• Professional dress (business casual) and strong communication skills
