Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide skilled physical therapy services to home health patients in a 60-mile radius of Valley City
Develop and implement individualized treatment plans for patients recovering from surgeries, falls, and other conditions
Collaborate with interdisciplinary team to ensure coordinated care
Document patient progress using HomeCare/HomeBase EMR system
Educate patients and families on home exercise programs and fall prevention

Qualifications

Active ND Physical Therapy License
Current BLS certification
Minimum 1 year of experience in home health
Valid Driver's License and Auto Insurance required
Must be able to cover a 60-mile radius around Valley City

Education

Graduate of an accredited Physical Therapy program

Knowledge & Skills

Strong clinical skills in physical therapy assessment and treatment
Experience with home health documentation and HomeCare/HomeBase EMR
Excellent communication and patient education skills
Ability to work independently and manage a caseload of 5-7 patients per day
