Location: Louisville, KY

Start Date: 06-29-2026

End Date: 09-28-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, with rotating weekend shifts

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct comprehensive home health evaluations and assessments
Develop and implement individualized physical therapy treatment plans
Perform start of care visits, recertifications, and discharges
Treat patients with diagnoses including CHF, orthopedic conditions, and pneumonia
Educate patients and families on therapeutic exercises and home safety
Collaborate with clinical managers and educators for patient care coordination

Qualifications

Active Physical Therapist (PT) license in Kentucky
Current BLS certification
Home health experience preferred but not required
New graduates welcome to apply
Comfortable using provided phone and tablet for documentation

Knowledge & Skills

Strong clinical knowledge in orthopedic and cardiopulmonary rehabilitation
Excellent communication and patient education skills
Ability to work independently in a home health setting
Time management and organizational abilities
