Shift Type: Monday-Friday, Day shifts

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide physical therapy services to patients in their homes
Conduct start-of-care visits for orthopedic patients
Develop and implement individualized treatment plans
Collaborate with nursing and other therapy staff
Document patient progress using HomeCare HomeBase EMR
Educate patients and families on therapeutic exercises

Qualifications

Current California Physical Therapy license
BLS certification required
Home Health experience preferred but will consider travelers with less experience
Strong communication and organizational skills

Knowledge & Skills

Knowledge of orthopedic rehabilitation and geriatric care
Proficiency in HomeCare HomeBase (HCHB) EMR
Ability to work independently and manage a caseload
Understanding of Medicare home health regulations
