Location: Santa Maria, California

Start Date: 06-17-2026

End Date: 09-16-2026

Contract Type: Contract

Shift Type: Flexible schedule with majority of work hours Monday-Friday, 8:00 AM - 5:00 PM; weekend requirement every 3rd weekend.

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Provide physical therapy services to patients in a home health setting
Develop and implement individualized treatment plans
Conduct comprehensive assessments including Oasis documentation
Collaborate with interdisciplinary team to optimize patient outcomes
Educate patients and families on therapeutic exercises and home safety
Monitor patient progress and adjust care plans as needed

Qualifications

Active Physical Therapist license in California
BLS certification required
Minimum of one year of physical therapy experience
Experience with Oasis documentation required
Home health experience preferred but not required

Knowledge & Skills

Knowledge of home health regulations and Oasis
Strong clinical assessment and treatment skills
Excellent communication and interpersonal abilities
Proficiency with Cerner EMR
Ability to work independently and manage a diverse caseload
