Contract Type: Travel Contract

Shift Type: Monday-Friday, Day shift (no weekends, no holidays; specific times not provided but typical home health hours)

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct initial assessments and start of care (SOC) visits for orthopedic and geriatric patients
Develop and implement individualized physical therapy plans to improve mobility and function
Collaborate with nurses, physicians, and home health aides to ensure comprehensive patient care
Educate patients and families on exercise programs and fall prevention strategies
Document patient progress in HCHB (HomeCare HomeBase) accurately and timely
Manage a daily caseload of approximately 5 visits, covering Tehama County area

Qualifications

Active Physical Therapist (PT) license in California
BLS certification (American Heart Association)
Minimum of 2 years of home health experience preferred; will consider less with strong home health background
Comfortable with daily travel (approx. 40 miles/day) and using a tablet for EMR charting
Excellent communication and collaboration skills

Knowledge & Skills

Experience with HomeCare HomeBase (HCHB) electronic medical record system
Knowledge of orthopedic rehabilitation and geriatric comorbidities (CHF, COPD)
Ability to work independently and manage a diverse patient caseload
Strong clinical reasoning and problem-solving skills
