Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct comprehensive initial evaluations (Start of Care visits) for patients with orthopedic and geriatric conditions
Deliver skilled physical therapy interventions in a home health setting, focusing on improving mobility and independence
Collaborate with nurses, occupational therapists, and speech therapists to create holistic care plans
Document patient progress in HCHB (HomeCare HomeBase) electronic medical record
Educate patients and caregivers on exercise programs and safety strategies

Qualifications

Current California Physical Therapy license
BLS certification from the American Heart Association
Home health experience required (new grads with home health rotations considered)
Willing to commute up to 40 miles daily across Tehama County

Knowledge & Skills

Proficiency with HCHB (HomeCare HomeBase) EMR
Ability to manage a caseload of 5 visits per day efficiently
Strong clinical reasoning for orthopedic and multi-morbidity patients
Excellent communication and time management skills
