Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in their homes across Tehama County
Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
Help patients recover from joint replacements and manage chronic conditions like CHF and COPD
Complete approximately 5 meaningful visits each day while building lasting patient relationships
Document care using HomeCare HomeBase EMR on the tablet provided by the facility

Qualifications

Current Physical Therapy license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years experience if they have Home Health background
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Strong assessment and treatment planning skills for orthopedic and geriatric patients
Comfortable working independently in home settings
Excellent communication and documentation abilities
Familiarity with HomeCare HomeBase EMR is a plus
