Physical Therapist

Location: Franconia, NH

Contract Type: Perm

Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day); full-time 40 hours/week with rotating weekends

Hours per Week: 40

Candidate Pay Rate: $41 per hour

Roles & Responsibilities

Evaluate and assess patients to create individualized physical therapy plans
Implement evidence-based treatment interventions to restore mobility and function
Document patient progress accurately and communicate updates with the care team
Collaborate with multidisciplinary staff at Lafayette Center / Powerback to coordinate care
Educate patients and families on home programs and strategies for long-term recovery
Participate in discharge planning and contribute to meaningful outcomes for patients

Qualifications

Demonstrated experience in Physical Therapy (PT) practice
Comfortable providing care in an outpatient/rehab-focused setting at the Lafayette Center / Powerback facility
Strong clinical documentation and communication skills
Ability to work the posted schedule (Monday–Friday with rotating weekends) and collaborate with a team

Knowledge & Skills

Skilled in patient assessment and development of individualized treatment plans
Proficient in therapeutic exercise, gait training, balance and functional mobility interventions
Knowledge of documentation best practices and outcome tracking
Excellent interpersonal skills and patient-centered approach
