Contract Type: Per-Diem

Shift Type: 8.5-hour shifts; specific start/end times and days to be confirmed

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents within the scope of LPN practice
Administer medications and treatments accurately and document per facility policy
Monitor and record vital signs, reporting significant changes to the RN or provider
Assist with daily living activities while promoting resident dignity and independence
Collaborate with the interdisciplinary team to support individualized care plans
Communicate clearly with residents and families about care and progress
Maintain a safe, clean environment and follow infection control protocols

Qualifications

Active LPN license in Pennsylvania (required)
Current BLS/CPR certification (or willingness to obtain)
Comfortable with medication administration and basic nursing procedures
Strong communication skills and the ability to work well in a team
Background check and required health screenings per facility policy

Education

Diploma in Practical Nursing or equivalent from an accredited program

Knowledge & Skills

Patient assessment and basic clinical documentation
Medication administration and adherence to safety protocols
Wound care and ADL assistance knowledge
Infection control and standard precautions
Familiarity with electronic health records or charting systems
Team collaboration and effective communication with residents and families
