Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents, focusing on safety and dignity
Administer medications and treatments accurately and in a timely manner
Monitor and record vital signs and patient conditions, escalating concerns to nursing leadership
Collaborate with nurses, therapists and care team members to develop and follow individualized care plans
Maintain accurate documentation in the resident chart and follow facility policies
Support a positive resident experience through effective communication with families and team members

Qualifications

Current and active LPN license for the state of Pennsylvania (required)
Basic Life Support (BLS) certification (or willingness to obtain)
Prior experience in long-term care or skilled nursing is preferred
Strong clinical assessment skills and attention to detail
Reliable, professional, and able to work effectively as part of a multidisciplinary team

Education

Completion of an accredited Practical Nursing (LPN) program
High school diploma or GED

Knowledge & Skills

Patient assessment and monitoring
Medication administration and documentation
Wound care basics and ADL support
Familiarity with electronic medical records and charting
Effective communication and teamwork skills
