Roles & Responsibilities

Provide compassionate direct patient care that keeps residents safe and comfortable
Administer medications and treatments accurately and document appropriately
Perform routine assessments and promptly report changes in patient condition
Collaborate with RNs, physicians, and interdisciplinary team members to deliver coordinated care
Maintain accurate, timely charting in the facility’s record system
Support patient mobility, hygiene, and daily living needs with dignity and respect

Qualifications

Current LPN license in Pennsylvania (or compact license where applicable)
Basic Life Support (BLS) / CPR certification or willingness to obtain
Must pass background check and meet facility immunization requirements
Strong communication skills and ability to work effectively in a team
Comfortable working 8-hour shifts and providing hands-on patient care

Education

Licensed Practical Nurse (LPN) diploma or equivalent; current PA LPN license required

Knowledge & Skills

Solid knowledge of medication administration and basic nursing procedures
Patient assessment and monitoring skills with attention to detail
Familiarity with electronic medical records (EMR) and accurate documentation
Good time management and ability to prioritize in a busy clinical setting
Compassionate bedside manner and strong interpersonal skills
