Roles & Responsibilities

• Provide compassionate, high-quality direct patient care in accordance with care plans and facility policies
• Administer medications and treatments safely and accurately, documenting per protocol
• Monitor patient status and communicate changes promptly to the care team
• Assist with ADLs and provide patient education to promote recovery and independence
• Maintain clear, timely charting in the electronic health record
• Collaborate with nurses, physicians, and support staff to deliver coordinated care
• Support a positive, person-centered environment that respects resident dignity

Qualifications

• Current, active LPN license in Pennsylvania (required)
• Basic Life Support (BLS) certification or the ability to obtain prior to start
• Comfortable administering medications and performing routine nursing procedures
• Strong communication skills and a team-oriented attitude
• Ability to pass background check and drug screening as required by the facility
• Previous experience in long-term care, rehab, or acute care is a plus

Education

• Practical Nursing diploma or equivalent vocational nursing education

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and infection control principles
• Patient assessment and vital signs monitoring
• Familiarity with electronic health records (EHR) and accurate charting
• Time management, prioritization, and delegation within a care team
• Empathy, patient advocacy, and strong interpersonal skills
