Roles & Responsibilities

Provide compassionate, resident-centered nursing care that promotes dignity and comfort
Administer medications and treatments accurately and document per facility policies
Monitor and record vital signs, recognize changes in condition, and escalate concerns
Collaborate with nurses, CNAs, therapists and families to support individualized care plans
Maintain infection control and safety standards to protect residents and staff
Participate in admissions, discharges and care plan meetings to ensure continuity of care

Qualifications

Current, unrestricted Pennsylvania LPN license (required)
Basic Life Support (BLS) certification from American Heart Association, or willingness to obtain
Ability to pass background check and meet facility onboarding requirements
Experience in long-term care or skilled nursing preferred but not required
Strong communication skills and a teamwork-oriented mindset

Education

Completion of an accredited Practical Nursing (LPN) program or equivalent

Knowledge & Skills

Resident assessment and monitoring (vitals, skin checks, ADLs)
Medication administration and accurate clinical documentation
Familiarity with electronic health records/charting
Infection control practices and safe patient handling
Time management, prioritization and multitasking during busy shifts
