Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents with professionalism and warmth
Administer medications and treatments accurately and document according to facility policy
Monitor resident condition and report changes promptly to the care team
Collaborate with nurses, CNAs, therapists, and families to create person-centered care plans
Maintain clear, timely charting and use facility systems to document interventions
Support a safe, clean environment and follow infection control and safety protocols
Participate in shift handoffs and contribute to a positive team culture at Autumn Lake Healthcare

Qualifications

Active LPN license in Connecticut (required)
Current BLS/CPR certification (or willing to obtain)
Comfortable with medication administration, basic wound care, and resident assessments
Ability to pass background check and meet facility onboarding requirements
Prior long-term care or skilled nursing experience is a plus but not strictly required

Education

Graduation from an accredited Practical Nursing program or equivalent vocational training

Knowledge & Skills

Strong patient care and bedside skills
Accurate medication administration and documentation
Basic assessment skills and recognizing changes in condition
Familiarity with electronic medical records or willingness to learn
Teamwork, clear communication, and a resident-first attitude
