Roles & Responsibilities

Provide safe, compassionate hands-on nursing care to residents
Administer medications and treatments according to care plans
Document patient status, interventions and outcomes accurately
Collaborate with nurses, therapists and support staff for quality care
Monitor residents for changes and escalate concerns promptly
Support resident activities of daily living and promote dignity

Qualifications

Valid LPN license in Pennsylvania required
Current CPR/BLS certification or willingness to obtain
Background check and required health screenings completed prior to start
Comfortable working in a skilled nursing or long-term care setting
Strong communication skills and a team-oriented attitude
Previous LPN experience preferred but new grads with clinical experience encouraged to apply

Education

Practical Nursing diploma or equivalent (LPN program completion)

Knowledge & Skills

Medication administration and proper charting practices
Fundamentals of wound care, vital signs and basic assessments
Familiarity with long-term care documentation and care plans
Ability to prioritize, multitask and remain calm in busy environments
Patient-centered care approach and strong interpersonal skills
