Roles & Responsibilities

• Provide compassionate, safe nursing care to patients as an LPN, focusing on dignity and comfort
• Administer medications and treatments per facility policies and physician orders
• Monitor patients’ conditions, record vital signs, and report changes promptly
• Collaborate closely with RNs and interdisciplinary team members to support care plans
• Maintain accurate, timely documentation in the patient record
• Support patients and families with education, reassurance, and clear communication
• Follow infection control standards and facility protocols to protect patients and staff

Qualifications

• Valid Licensed Practical Nurse (LPN) license (state-appropriate) — required
• Must be local to the Rutland, VT area and have reliable personal transportation (LOCAL ONLY)
• Ability to work the assigned 2:00 PM - 10:30 PM shift and meet contractual RTO requirements
• Current BLS/CPR as required by facility (verify during onboarding)
• Strong communication skills and the ability to collaborate with a multidisciplinary team

Education

• Graduate of an accredited practical nursing program and current LPN licensure

Knowledge & Skills

• Solid practical nursing skills: medication administration, wound care, vital signs, ADL assistance
• Good clinical assessment and patient observation skills
• Comfortable with accurate, timely charting and documentation
• Team-oriented mindset and ability to follow care plans set by RNs and providers
• Ability to prioritize tasks in a busy clinical environment while maintaining compassion
