Roles & Responsibilities

Provide compassionate direct care to residents as a Licensed Practical Nurse
Administer medications and treatments accurately and timely
Monitor patient condition and report changes to the care team
Document assessments, interventions, and patient responses in the chart
Collaborate with nurses, therapists, and support staff to deliver coordinated care
Support admission, discharge, and care planning activities
Maintain a safe, clean, and respectful environment for residents

Qualifications

Valid LPN license in Pennsylvania (required)
Current BLS/CPR certification or willingness to obtain
Up-to-date immunizations and TB screening per facility policy
Strong communication and interpersonal skills
Comfortable with electronic medical records and standard charting
Previous experience in long-term care or skilled nursing helpful but not required

Education

LPN diploma, certificate, or vocational nursing program completion

Knowledge & Skills

Medication administration and basic clinical procedures
Wound care fundamentals and monitoring for changes
Vital signs assessment and documentation
Time management and prioritization in a busy clinical setting
Teamwork, delegation, and clear communication with interdisciplinary staff
Basic computer skills for charting and electronic documentation
