Roles & Responsibilities

• Coordinate comprehensive discharge planning to ensure safe, timely transitions from hospital to next level of care
• Conduct utilization review and apply Milliman guidelines to support appropriate resource use
• Develop, communicate, and update individualized plans of care while collaborating with the care team
• Perform needs assessments, place DME orders, and arrange community supports to remove barriers to discharge
• Document accurately and efficiently in Epic to maintain clear patient records and compliance
• Serve as a liaison between patients, families, clinical teams, and community partners to optimize outcomes
• Float within case management services as needed to cover gaps and support continuity of care
• Support regulatory compliance (CMS, HIPAA) and contribute to quality initiatives

Qualifications

• AK RN license in hand required for auto-offer (screen for pending; local candidates not accepted — permanent address must be 50+ miles from facility)
• Minimum 1 year of RN experience; acute care experience required
• Epic experience required (hardstop) — strong charting skills a must
• ACM (Case Management) certification preferred
• BLS preferred
• Ability to work every-other-weekend rotation (or 4 weekend days per 4-week schedule) and float within scope
• Comfort working in a high-volume acute hospital (Magnet, Level II Trauma) and collaborating with interdisciplinary teams

Knowledge & Skills

• Expertise in case management, care coordination, and discharge planning
• Familiarity with utilization review processes and Milliman guidelines
• Proficient charting and documentation in Epic
• Working knowledge of CMS regulations and HIPAA compliance
• Experience ordering DME and completing needs assessments
• Comfortable supporting diverse patient populations from neonates to geriatrics
• Good communication and teamwork skills for interdisciplinary collaboration
• Basic computer skills, including Microsoft Office/Teams
