Shift Type: Rotating shifts with weekend requirement every 4 weeks and all minor holidays plus 2 major holidays worked

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Provide compassionate end-of-life care to hospice patients in their homes and facilities
Manage a caseload of 12-14 patients with 4 visits daily or 2 start-of-care assessments
Build meaningful relationships with patients and families during their most important journey
Collaborate with interdisciplinary teams to ensure comprehensive, dignified care
Document care using HCHB Home Care Home Base EMR system

Qualifications

Active Registered Nurse license in California
Current BLS certification
Minimum 1 year of hospice experience required
Strong communication and compassionate bedside manner

Knowledge & Skills

Hospice care expertise and end-of-life support
Home care and facility-based patient visits
HCHB Home Care Home Base EMR proficiency
Patient assessment and care planning skills
Ability to work independently in various home settings
