Roles & Responsibilities

• Perform initial patient evaluations and regularly re-evaluate nursing needs to ensure optimal care
• Initiate, implement and revise individualized plans of care for home health, hospice, and palliative patients
• Provide skilled nursing interventions, including preventive and rehabilitative procedures
• Prepare clear, timely clinical and progress notes to support continuity of care
• Coordinate services across disciplines and communicate patient status changes to physicians and team members
• Counsel patients and families to support coping, decision-making, and care goals
• Participate in in-service programs to maintain clinical excellence and share best practices

Qualifications

• Registered Nurse (RN) — current/active licensure (RN) required
• Experience in Home Health, Hospice, Palliative Care and/or Case Management
• Proven skills in clinical documentation, patient assessment and care planning
• Ability to perform and document preventive and rehabilitative nursing interventions
• Comfort with on-call duties and flexible scheduling inherent to community-based care
