Roles & Responsibilities

• Evaluate patients to establish functional deficits and create individualized treatment plans
• Provide skilled therapeutic interventions in outpatient and assisted living settings
• Deliver hands-on care and progressive exercise programs to restore mobility and independence
• Document patient progress clearly and timely in the facility’s clinical records
• Collaborate with nurses, aides, and interdisciplinary team members to coordinate patient care
• Educate patients and caregivers on home programs, safety, and fall prevention
• Monitor outcomes and adjust treatment plans to achieve meaningful functional gains
• Ensure compliance with facility policies and regulatory standards

Qualifications

• Licensed Physical Therapist (PT) — ability to practice in North Carolina required
• DPT preferred (core skills list includes DPT) — new grads are welcome
• Experience or comfort working in Outpatient and Skilled Nursing Facility (SNF) settings is a plus
• Current BLS/CPR certification (or willingness to obtain)
• Strong documentation and communication skills
• Ability to pass background checks and meet facility onboarding requirements

Education

• DPT or equivalent degree in Physical Therapy (DPT preferred)
• New graduates accepted

Knowledge & Skills

• Outpatient evaluation and treatment planning
• Skilled nursing facility (SNF) care knowledge and transitions
• Gait training, balance/fall prevention, and mobility restoration
• Manual therapy and therapeutic exercise techniques
• Patient and family education with a focus on functional outcomes
• Proficient clinical documentation and familiarity with electronic health records
• Interdisciplinary collaboration and care coordination
• Time management and caseload prioritization
