Roles & Responsibilities

• Evaluate patients to establish functional limitations, goals, and individualized treatment plans
• Develop and implement evidence-based therapeutic interventions to improve mobility and independence
• Monitor patient progress and modify plans of care to achieve measurable outcomes
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and case managers for coordinated care
• Document assessments, interventions and patient progress accurately and in a timely manner
• Educate patients and families on home programs, safety, and strategies to maintain gains after discharge
• Participate in team meetings and contribute to quality improvement initiatives

Qualifications

• Licensed Physical Therapist in New Jersey or eligible to obtain state licensure
• Doctor of Physical Therapy (DPT) preferred; bachelor’s or master’s in physical therapy considered
• Current CPR/BLS certification (or obtain upon hire)
• Experience in inpatient rehabilitation or post-acute care is preferred
• Strong clinical decision-making and patient-centered care skills
• Able to pass standard background checks and meet facility onboarding requirements

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor’s or Master’s in Physical Therapy accepted

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, gait training, and balance interventions
• Proficient with documentation and electronic health record charting
• Familiarity with rehabilitation equipment and safe patient-handling techniques
• Excellent communication and interdisciplinary collaboration skills
• Ability to prioritize caseloads and manage time effectively in a fast-paced environment
