Roles & Responsibilities

• Evaluate patients using evidence-based assessment tools to create individualized care plans
• Develop and implement treatment plans that promote functional independence and safety
• Provide skilled therapy across inpatient, swing bed, outpatient, and skilled nursing settings
• Document clinical findings, progress, and discharge planning accurately in the EMR
• Collaborate with physicians, nurses, and interdisciplinary team members to coordinate care
• Educate patients and families on treatment goals, home exercise programs, and safety strategies
• Demonstrate strong clinical reasoning to adapt care for diverse diagnoses and populations
• Participate in quality improvement and professional development initiatives

Qualifications

• Active California Physical Therapy License (in good standing)
• Successful completion of the National Physical Therapy Examination (NPTE)
• Current AHA BLS (Basic Life Support) certification
• 1+ year of experience in acute care, swing bed/skilled nursing, or outpatient settings
• Proficiency with EMR systems and general computer skills
• Strong communication, documentation, and clinical reasoning abilities
• Commitment to ongoing professional development and high attention to detail

Education

• Bachelor's, Master's, or Doctor of Physical Therapy (DPT) from an accredited program

Knowledge & Skills

• Patient evaluation and comprehensive treatment planning
• Inpatient, outpatient, swing bed, and skilled nursing care
• Skilled nursing care practices and transitions of care
• Electronic medical record (EMR) proficiency and accurate documentation
• Clinical reasoning and problem-solving for complex patient needs
• Basic Life Support (BLS) knowledge and safety protocols
• Effective communication with interdisciplinary teams and patients
• Computer proficiency for documentation and care coordination
