Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Deliver personalized physical therapy services directly in patients' homes across your assigned region
Develop and implement individualized treatment plans for patients recovering from strokes, joint replacements, and spinal injuries
Collaborate with an interdisciplinary team including OT and Speech therapists to optimize patient outcomes
Complete OASIS documentation to ensure compliance with government-mandated requirements
Educate patients and families on exercises and techniques to support recovery and independence

Qualifications

Active Physical Therapist license in California
Minimum 1 year of experience as a Physical Therapist
BLS certification required
Experience with OASIS documentation required
Home health experience preferred but not required

Knowledge & Skills

Strong assessment and clinical reasoning skills
Excellent communication and patient education abilities
Comfortable working independently in home settings
Proficient with Cerner EMR system
Ability to travel within assigned geographical region
