Roles & Responsibilities

• Plan and conduct individualized treatment programs to restore and improve physical, social and mental function
• Complete thorough evaluations to identify patient needs and ensure quality standards are met
• Develop and implement evidence-based treatment plans and therapy interventions
• Create and communicate clear discharge and care plans to patients, families and the care team
• Complete accurate, timely clinical documentation using required systems (Homecare Home Base / OASIS)
• Collaborate as a co-leader in interdisciplinary patient care to achieve the best outcomes
• Supervise and mentor assigned therapy assistants to support high-quality care
• Meet productivity and quality expectations while prioritizing patient safety and outcomes

Qualifications

• Active Hawaii State PT license required (active at submittal — no exceptions)
• Minimum 2 years of PT experience (required); at least 1 year of home health PT experience (required)
• Experience with Homecare Home Base and OASIS (required)
• BLS (AHA) and CPR certification required
• Bachelor’s degree in Physical Therapy or related field required; Master’s degree preferred
• Travel/contract experience preferred
• Able to complete clinical documentation within established deadlines and meet productivity expectations (25 points/week)

• Bachelor's degree in Physical Therapy or related field (required)
• Master's degree in a related field (preferred)

• Patient assessment and comprehensive evaluation skills
• Treatment planning and delivery of evidence-based therapy interventions
• Strong clinical documentation practices and familiarity with Homecare Home Base and OASIS
• Discharge planning and care coordination across the interdisciplinary team
• Home health care workflows and safety practices
• Supervision and mentoring of therapy assistants
• Effective communication with patients, families and clinical teams
