Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Restore and maximize patient function through personalized occupational therapy programs
Evaluate and treat patients to enhance activities of daily living and prevent further limitations
Help clients achieve their highest performance levels within their unique capabilities
Collaborate with the Pathways Home Health & Hospice team to deliver compassionate care

Qualifications

Active Occupational Therapist (OT) license in California
Experience in home health settings preferred
BLS certification
Passionate about patient-centered rehabilitation

Education

Knowledge & Skills

Expertise in medically prescribed OT programs
Strong skills in patient evaluation and treatment planning
Knowledge of home health territory including San Jose, Sunnyvale, and surrounding areas
Excellent communication and empathy for building client trust
