Roles & Responsibilities

• Answer patient calls from hospitals, pharmacies, and offices, directing info to the right providers swiftly
• Assist providers during exams, treatments, and procedures with skill and care
• Perform patient follow-up calls on test results and care plans as directed
• Collect vital health data, spotting normal and abnormal findings accurately
• Coordinate seamless follow-up care, scheduling tests and referrals promptly
• Document assessments, care, and data concisely in medical records
• Handle lab duties like phlebotomy, specimen collection, and basic waived testing
• Submit prescriptions and refills to pharmacies efficiently
• Educate patients and families on care plans, documenting every step

Qualifications

• 2+ years of LPN experience required
• Experience in a doctor's office highly preferred
• Active Illinois LPN license via Nursys
• Active American Heart Association BLS certification
• Skills checklist current to 1 year for LPN
• 2 supervisory references with employment dates
