Hours per Week: 40

Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Coordinate seamless patient care transitions that make a real difference
Master complex discharge planning to ensure safe patient journeys home
Navigate Meditech and Midas systems with confidence to optimize workflows
Collaborate with RN CM and SW CM teams for holistic patient support
Advocate for patients managing ratios up to 25:1 with skill and compassion

Qualifications

Active RN license in good standing
At least 1 year of case management experience
Proficiency in Midas and Meditech systems required
Florida RN license or ability to obtain quickly for travel nurses

Education

RN degree from accredited nursing program

Knowledge & Skills

Expertise in RN Case Management
Hands-on Midas experience
Meditech system navigation
Complex discharge planning skills
