Roles & Responsibilities

Provide compassionate direct patient care and monitor patients' status with attention and empathy
Administer medications and treatments as directed, following facility protocols
Collaborate with RNs, physicians, and interdisciplinary teams to deliver coordinated care
Document patient care activities accurately and timely in the medical record
Assist with admissions, discharges, and transfers while maintaining patient comfort
Support patient education and family communication to promote positive outcomes
Follow infection control and safety procedures to maintain a clean, safe environment

Qualifications

Active LPN license required (Pennsylvania licensure preferred)
Current BLS/CPR certification or willingness to obtain upon hire
Completion of an accredited practical nursing program
Strong communication skills and a patient-centered approach
Ability to pass required health screenings and background checks
Previous long-term care or acute care experience is a plus

Education

Diploma or certificate from an accredited practical nursing program
Completion of state LPN licensure requirements

Knowledge & Skills

Solid clinical assessment and basic nursing skillset
Medication administration and documentation best practices
Familiarity with infection control standards and safety protocols
Comfortable working in a fast-paced clinical environment
Team-oriented mindset with good time management and organization
