Roles & Responsibilities

Provide compassionate, hands-on nursing care to assigned residents and patients
Administer medications and treatments accurately and safely
Monitor patient condition, document changes, and communicate effectively with the care team
Assist with daily living activities while promoting dignity and independence
Follow facility policies, infection control procedures, and safety protocols
Support transitions of care by coordinating with other providers and family members

Qualifications

Current LPN license in Pennsylvania (or compact/state-appropriate licensure as required)
CPR/BLS certification (current)
Up-to-date immunizations and background check clearance
Comfortable with medication administration, charting, and basic assessments
Excellent communication skills and a patient-centered mindset
Prior long-term care or facility experience preferred but motivated new grads are welcome to apply

Education

Diploma or certificate in Practical Nursing (LPN) or equivalent

Knowledge & Skills

Knowledge of basic nursing assessments and documentation
Skilled in medication administration and safe handling
Familiarity with infection control and safety procedures
Strong teamwork and communication skills
Ability to manage time and prioritize in a fast-paced clinical setting
Compassionate patient care with attention to detail
