Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately and on schedule
Document patient status, interventions and outcomes in the electronic health record
Monitor patient condition and promptly report changes to the nursing team and providers
Collaborate with nurses, therapists and support staff to deliver coordinated care
Assist with admissions, discharges and safe patient transfers
Educate patients and families about care plans, medications and follow-up

Qualifications

Current Licensed Practical Nurse (LPN) license in Pennsylvania required
Current BLS/CPR certification required
Completion of an accredited practical/vocational nursing program
Background check, immunizations and TB screening as required by the facility
Strong communication skills and the ability to work well in a team
Previous LPN experience in long term care or similar settings preferred

Education

Graduate of an accredited Practical Nursing (LPN) or Vocational Nursing program

Knowledge & Skills

Medication administration and basic clinical procedures
Patient assessment and vital signs monitoring
Basic wound care and infection control practices
Accurate and timely electronic charting
Time management and prioritization in a fast-paced setting
Compassionate patient- and family-centered care
