Roles & Responsibilities

Provide compassionate, resident-centered nursing care as an LPN
Administer medications and treatments accurately and safely
Monitor resident condition and communicate changes to the care team
Document care activities clearly and timely in resident records
Collaborate with RNs, CNAs, and interdisciplinary staff to deliver excellent care
Support resident dignity and independence through personalized care plans

Qualifications

Current Licensed Practical Nurse (LPN) license in Pennsylvania (required)
Basic Life Support (BLS) certification or willingness to obtain
Demonstrated ability to work effectively in a team-focused care setting
Strong communication and documentation skills
Reliable, compassionate, and resident-centered approach to care

Education

LPN diploma or certificate from an accredited program
High school diploma or GED

Knowledge & Skills

Knowledge of basic nursing practices and medication administration
Familiarity with long-term care or skilled nursing environments
Ability to accurately chart and follow care plans
Strong interpersonal skills and ability to engage residents respectfully
Basic clinical assessment and infection control practices
