Roles & Responsibilities

• Provide direct patient care with compassion and clinical accuracy
• Administer medications and treatments per physician orders and facility policy
• Perform routine assessments and communicate changes in patient condition promptly
• Maintain clear, timely documentation in the patient record and EMR
• Collaborate with nurses, therapists, and interdisciplinary team members to create patient-focused care plans
• Assist with activities of daily living while promoting patient dignity and independence
• Adhere to infection control, safety protocols, and facility policies to protect patients and staff

Qualifications

• Current Licensed Practical Nurse (LPN) license in Connecticut, or eligible to obtain one
• Basic Life Support (BLS) certification required or obtainable upon hire
• Comfortable with medication administration, wound care basics, and routine clinical tasks
• Clear background check and up-to-date immunizations per facility requirements
• Strong communication skills and a team-first attitude

Education

• Vocational nursing diploma or certificate (LPN/LVN program) required

Knowledge & Skills

• Medication administration and documentation accuracy
• Patient assessment and vital sign monitoring
• Basic wound care and ADL assistance
• Electronic medical record (EMR) charting and documentation
• Infection prevention and safety best practices
• Time management and prioritization in a busy long-term care setting
• Effective teamwork and patient-family communication
