Roles & Responsibilities

• Provide compassionate, resident-centered nursing care tailored to older adults
• Administer medications and treatments accurately and document in the medical record
• Monitor and record vital signs, reporting significant changes promptly
• Assist residents with activities of daily living while promoting independence
• Collaborate with RNs and interdisciplinary team members to support care plans
• Maintain clear, timely documentation and communicate effectively with families
• Support a safe, clean environment and adhere to infection control standards

Qualifications

• Current Licensed Practical Nurse (LPN) license valid in New York State
• BLS/CPR certification (or willing to obtain upon hire)
• Experience or strong interest in geriatric care or long-term care settings preferred
• Good communication skills and ability to work within a team
• Up-to-date immunizations and ability to comply with facility onboarding requirements

Education

• LPN diploma or equivalent completion of an accredited practical nursing program

Knowledge & Skills

• Working knowledge of geriatric care principles and long-term care best practices
• Medication administration and basic clinical assessment skills
• Familiarity with electronic medical records and accurate documentation
• Strong interpersonal skills for resident- and family-centered interactions
• Ability to prioritize tasks in a fast-paced care environment
