Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in a long-term care setting
Administer medications and treatments accurately and document per facility policy
Monitor patient condition, report changes to the care team, and escalate concerns
Collaborate with nurses, therapists, and support staff to create individualized care plans
Maintain clear, timely charting and ensure regulatory compliance with documentation
Support resident dignity and independence through respectful, person-centered care

Qualifications

Active LPN license in good standing (or compact license where applicable)
Current CPR/BLS certification (or willingness to obtain upon hire)
Previous experience in long-term care, skilled nursing, or similar setting preferred
Ability to pass background check, drug screen, and required health screenings
Strong communication skills and a team-oriented attitude

Education

LPN diploma or certificate from an accredited nursing program

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care, ADL assistance, and infection control principles
Vital signs assessment and monitoring clinical changes
Electronic medical record (EMR) charting familiarity
Time management, prioritization, and the ability to work in a fast-paced environment
Compassionate resident interaction and family communication
