Roles & Responsibilities

• Provide compassionate direct patient care with attention to safety, dignity and comfort
• Administer medications and treatments accurately and document care in the patient chart
• Perform routine assessments, monitor patient status and report changes to the care team
• Assist with wound care, dressing changes and basic procedures as delegated
• Collaborate with nurses, therapists and support staff to create and follow individualized care plans
• Educate patients and families on care routines, medications and discharge instructions
• Contribute to a positive unit culture by mentoring peers and supporting new team members

Qualifications

• Active LPN license (eligible for or issued in New Mexico preferred)
• Current BLS/CPR certification required
• Experience in long-term care or skilled nursing is a plus but strong clinical fundamentals matter most
• Ability to pass background checks and required health screenings
• Strong communication skills and a team-first attitude

Education

• Completed practical nursing program (LPN diploma or certificate)

Knowledge & Skills

• Solid clinical assessment and basic nursing skills
• Medication administration and accurate charting
• Familiarity with long-term care workflows and documentation systems
• Comfort with time management during busy, extended day shifts
• Good interpersonal skills for patient/family education and teamwork
