Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents at the rehab and long-term care facility
• Administer medications and treatments accurately and safely according to facility protocols
• Assess resident condition, monitor changes, and escalate concerns to the care team
• Document care thoroughly in the medical record to ensure continuity and quality
• Collaborate with nurses, therapists, and support staff to create meaningful care plans
• Support residents with activities of daily living and promote a positive, dignified environment
• Assist with admissions, discharges, and transfers to keep operations running smoothly

Qualifications

• Active Licensed Practical Nurse (LPN) license — ready to practice
• Current BLS/CPR certification or willingness to obtain prior to start
• Experience in long-term care or rehabilitation preferred and valued
• Ability to pass background check, drug screen, and meet facility onboarding requirements
• Strong communication skills and a friendly, team-oriented attitude

Education

• Vocational nursing diploma or completion of an accredited LPN program
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic clinical assessment and monitoring of vital signs
• Wound care basics and infection control principles
• Familiarity with electronic health records and accurate charting
• Time management, prioritization, and teamwork in a fast-paced setting
• Compassionate resident engagement and customer-service mindset
