Roles & Responsibilities

• Conduct comprehensive speech-language evaluations to assess communication and swallowing needs
• Develop and implement individualized treatment plans that promote functional gains
• Deliver daily treatments and therapeutic interventions that improve patient outcomes
• Plan and coordinate safe, effective discharges in collaboration with the care team
• Document thoroughly and maintain productivity expectations (90% productivity requirement)
• Follow Continuum Rehabilitation Group (CRG) policies and procedures to ensure consistent, high-quality care
• Participate as a collaborative member of the Spanish Trails Wellness & Rehab therapy team

Qualifications

• 1-3 years of relevant experience (as listed in the job posting)
• Prior SNF (skilled nursing facility) experience strongly preferred
• Ability and commitment to be present in the building to receive guaranteed hours
• Willingness to follow CRG policies & procedures and meet productivity expectations
• Ability to work a contract assignment (13-week length)

Knowledge & Skills

• Speech-Language Pathology clinical skills
• Experience performing evaluations and developing treatment plans
• Daily treatment delivery for a variety of SNF patients
• Discharge planning and coordination
• Familiarity with documentation standards and productivity tracking
• Comfort working in a skilled nursing facility environment
