Roles & Responsibilities

• Evaluate and assess speech, language, cognitive-communication, and swallowing function for SNF residents
• Develop and implement individualized treatment plans that promote functional improvement
• Provide skilled therapy interventions, including dysphagia management when indicated
• Document timely, accurate clinical notes and therapy progress according to facility and CRG policies
• Collaborate with nursing, rehab team members, and family to coordinate care and maximize outcomes
• Meet productivity expectations and contribute positively to the therapy team culture
• Maintain professional appearance and follow facility dress code and policies

Qualifications

• Licensed SLP or credentialed speech-language pathologist as required by state regulations
• 1-3 years of clinical experience (as indicated in the order)
• Experience in skilled nursing facility or geriatric settings preferred
• Comfort with facility policies and productivity expectations (therapist productivity requirement noted)
• Ability to be on-site and meet guaranteed hours as required by the facility
• Willingness to follow Continuum Rehabilitation Group and facility policies and procedures

Knowledge & Skills

• Skilled in pediatric or adult speech-language pathology assessment and treatment relevant to SNF populations
• Competence in dysphagia evaluation and intervention techniques
• Strong documentation skills and familiarity with clinical productivity tracking
• Collaborative communication with interdisciplinary team members
• Ability to manage caseloads in a skilled nursing environment and prioritize resident-centered care
