Roles & Responsibilities

• Assess residents’ speech, language, and swallowing needs and develop person-centered treatment plans
• Deliver skilled SLP interventions in a skilled nursing facility to improve patient communication and safety
• Document progress and maintain timely, compliant clinical records
• Collaborate with nursing and therapy teams to coordinate care and support positive outcomes
• Meet facility productivity expectations (90%) while maintaining high-quality care
• Follow Continuum Rehabilitation Group policies & procedures and represent the facility professionally (black scrubs)

Qualifications

• 1–3 years of SLP experience (as stated in the job information)
• Experience or comfort working in a skilled nursing facility (SNF)
• Ability to meet the facility productivity requirement (90%)
• Willingness to work on-site and follow facility dress code and policies
• Strong communication and interdisciplinary collaboration skills

Knowledge & Skills

• Foundational Speech-Language Pathology knowledge and clinical skills
• Skilled nursing facility workflows and resident-centered care practices
• Efficient clinical documentation and time management to support productivity goals
• Collaborative teamwork with therapists, nurses, and facility staff
• Familiarity with employer policies & procedures (CRG P & P) and professional standards
