Roles & Responsibilities

• Conduct comprehensive speech-language evaluations to identify patient needs
• Create individualized treatment plans and carry out daily therapy sessions
• Lead and coordinate discharge planning to ensure safe, effective transitions
• Document care and progress in accordance with facility and CRG policies
• Collaborate with the therapy team and interdisciplinary staff to optimize outcomes
• Meet productivity expectations and contribute to a high-performing therapy program
• Maintain professional presence on-site (dress code: black scrubs)

Qualifications

• 1-3 years of clinical experience (as listed in the order)
• Prior SNF experience strongly preferred
• Demonstrated ability to perform evaluations, daily treatments, and discharge planning
• Comfortable meeting a 90% productivity requirement and following CRG policies
• Able and willing to be on-site in the facility to receive guaranteed hours
• Adheres to facility dress code (black scrubs) and professional standards

Knowledge & Skills

• Speech-Language Pathology clinical expertise
• Skilled in evaluations and treatment planning
• Experience providing daily treatments in a SNF setting
• Discharge planning and care coordination
• Good documentation practices and familiarity with facility policies
• Strong teamwork and patient-centered communication skills
