Candidate Pay Rate: $53 per hour

Roles & Responsibilities

• Provide compassionate, patient-centered hospice nursing care in homes, assisted living facilities, and nursing homes
• Assess and manage symptoms expertly, using strong clinical judgment and critical thinking
• Serve as the Primary RN for an assigned caseload and lead the interdisciplinary team for those patients
• Teach and coach patients and families to increase caregiver confidence and meet goals of care
• Document care clearly and coordinate with physicians, social workers, chaplains, and other team members
• Respond to changing patient needs, provide crisis support, and ensure dignity and comfort

Qualifications

• Active Registered Nurse (RN) license valid in Colorado
• BLS (Basic Life Support) certification required; hospice/palliative care certifications a plus
• Comfort and experience providing care in community settings (private homes, assisted living, nursing homes)
• Strong assessment, critical thinking, and patient/family teaching skills
• Excellent communication and interdisciplinary teamwork abilities
• Willingness to provide occasional weekend back-up and participate in on-call rotation

Education

• Active RN license in Colorado
• ADN or BSN preferred

Knowledge & Skills

• Expertise in symptom management and end-of-life care principles
• Skilled in comprehensive assessments and care planning
• Ability to teach and support caregivers with empathy and clarity
• Proficiency with clinical documentation and care coordination
• Comfort working independently in community settings and as a team leader
