Location: Red Bluff, California

Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Deliver personalized physical therapy care to patients in their homes across Tehama County
- Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
- Help patients recover mobility and independence after joint replacements and manage chronic conditions like CHF and COPD
- Complete approximately five meaningful patient visits each day using a company-provided tablet
- Collaborate with an Intake RN and office manager to coordinate seamless care plans
Qualifications
- Current Physical Therapist license in California
- BLS certification required
- Home Health experience preferred but candidates with less than two years will be considered if they have Home Health exposure
- Comfortable traveling within the service area and managing a daily mileage of about 40 miles
Knowledge & Skills
- Skilled in HomeCare HomeBase (HCHB) documentation
- Strong assessment and care-planning abilities for orthopedic and geriatric patients
- Excellent time-management and independent decision-making skills
- Friendly communicator who enjoys building rapport with patients and families
