Roles & Responsibilities
- Deliver personalized physical therapy to patients recovering from falls and fractures
- Complete Oasis Assessments and Start of Care evaluations with care and accuracy
- Build trust with elderly patients across four rural counties
- Track visits and time seamlessly using provided tablet technology
- Collaborate with a multidisciplinary team to support whole-patient recovery
Qualifications
- Active Physical Therapist license in Indiana
- Comfortable performing Oasis Assessments and Start of Care visits
- Reliable transportation for travel between rural communities
- Strong communication skills and compassionate bedside manner
