Roles & Responsibilities

• Coordinate and lead discharge planning to ensure safe, timely transitions from hospital to next level of care
• Provide care coordination and case management for a diverse acute care population across both Colby and Pacific campuses
• Perform utilization review and needs assessments, including ordering DME as indicated
• Document thoroughly and accurately in Epic to support continuity of care
• Collaborate with interdisciplinary teams, including Social Services, to create and execute patient-centered plans
• Float across hospital areas as needed to meet unit demands and support patient care
• Educate patients and families on discharge plans and community resources to promote positive outcomes

Qualifications

• Active RN licensure (WA) required; pending WA license accepted
• BLS certification required
• Minimum 1 year acute care experience required
• Demonstrated discharge planning/care coordination and case management experience
• Epic charting experience required
• Comfortable working in a Level II Trauma Center and with patients across the lifespan (neonates to geriatrics)
• Willingness to work every other weekend
• Locals not accepted; please include all RTO at time of submittal
• First-time travelers accepted

Knowledge & Skills

• Discharge planning and care coordination best practices
• Case management and utilization review processes
• Epic electronic health record proficiency
• Needs assessment and DME ordering
• Experience or comfort with Med Surg, LLOS, ED, and ICU settings (preferred)
• Strong clinical judgment, documentation, and communication skills
• Ability to collaborate effectively with interdisciplinary teams
