Roles & Responsibilities

Evaluate patients' mobility, strength, balance, and functional needs and develop individualized treatment plans
Provide hands-on and progressive therapeutic interventions to help patients regain independence in an SNF setting
Track patient progress, revise care plans as needed, and maintain clear, timely documentation
Collaborate with nurses, occupational therapists, speech therapists, and physicians to coordinate comprehensive care
Educate patients and families on home programs, safe transfers, and fall prevention strategies
Contribute to interdisciplinary meetings and help drive quality improvement initiatives

Qualifications

PT or DPT credential (PT/DPT listed as core skills)
Open to all experience levels, including new graduates (years of experience: any, including New Grad)
Comfortable working in a Skilled Nursing Facility (SNF) practice setting
Ability to complete facility onboarding and credentialing requirements

Education

DPT or equivalent Physical Therapy degree (new graduates welcome)

Knowledge & Skills

Experience or familiarity with geriatric rehabilitation and SNF-specific therapy approaches
Strong patient assessment and clinical reasoning skills
Proficient documentation and outcomes tracking for regulatory and clinical needs
Excellent communication and teamwork skills for interdisciplinary collaboration
Skilled in mobility, gait, balance, and therapeutic exercise interventions
