Roles & Responsibilities

• Provide skilled physical therapy evaluations and evidence-based treatment plans for adults in SNF and outpatient settings
• Float and assist between sister outpatient sites to ensure consistent, high-quality patient care
• Collaborate with interdisciplinary teams including nursing and therapy colleagues to optimize patient outcomes
• Document timely, clear, and compliant patient notes and progress toward goals
• Educate patients and families on home programs, safety, and functional goals
• Participate in team meetings and care planning to support smooth transitions and continuity of care

Qualifications

• Licensed Physical Therapist in Minnesota (or eligible to obtain state licensure)
• DPT or equivalent physical therapy degree (new graduates welcome — any experience level considered)
• Comfortable working in both SNF and outpatient environments and floating between sites
• Current CPR/BLS certification (or willingness to obtain)
• Strong communication and teamwork skills with a patient-centered approach

Education

• Doctor of Physical Therapy (DPT) or entry-level PT degree; new graduates accepted

Knowledge & Skills

• Skilled in physical therapy assessment, therapeutic exercise, manual techniques, and functional mobility training
• Knowledge of SNF and outpatient practice patterns, documentation standards, and regulatory requirements
• Ability to adapt treatment plans across different settings and patient populations
• Excellent interpersonal skills for patient education and interdisciplinary collaboration
• Organized documentation and time-management skills to handle a floating schedule
